The idea is that the coldness slows circulation in the head, which in turn slows the chemo from reaching the hair follicles. The cap is popular in Europe and just started being available in the United States.
Basically, the cap is shaped like a winter hat with ice packs similar to those used for knee injuries. The cap is left on for 30 minutes and swapped for another cap.
